Nurse’s Corner

Hello Girard Academic Music Program families.

My name is Danielle Noorlander-Quain, but please call me Nurse Dani. This is my eighth year in the district; my background is predominantly in pediatric and neonatal critical care. In addition, I also teach nursing students on the weekends at Drexel University.

I am available during normal school days and monitor my email regularly. Please email me if you have any questions or concerns I can assist with. Completed documents such as physicals, immunizations, dental forms, and emergency contact forms can be submitted via email. I am always available for urgent and emergent events.

I look forward to a healthy, well-balanced school year and getting to know all of you.

IMMUNIZATION REQUIREMENTS

State law requires all children entering school to meet specific immunization requirements. If the requirement is not met, your child may be excluded from school until the requirement is met. Click here for more information about immunization requirements.

MEDICATION DURING SCHOOL

Students are NOT allowed to have any medications on their person or in their school bag at any time unless approved by the school nurse. This includes asthma inhalers and over-the-counter medications.

Any medication (prescription and over-the-counter medicines) or treatment necessary during the school day requires the following:

  • The MED-1 form must be completed by the child’s medical provider, signed by the parent/guardian, and updated yearly.
  • The medications must be submitted directly to the school nurse in the original packaging from the pharmacy.
  • It is strongly recommended that AM medications are taken at home with parental supervision.
  • Medications that must be given during school hours will occur between 10:00 am through 1:49 PM.

MEDICAL CONDITION

For your child’s safety and well-being, please notify the school nurse of any health condition that might impact your child during school hours. We request that you complete Form S-865 every year and submit it to the school nurse as soon as possible. This form also includes signed permission for Tylenol (Acetaminophen), Motrin, (Ibuprofen), and emergency administration of epinephrine and albuterol inhaler.

PHYSICAL EXAMINATIONS

Students entering 6th and 11th grades will need a physical exam completed by their doctor for the 2026-2027 school year, when the time allows, over the summer break. The physical must be recent and within the last 6 months. If your child is due for a checkup soon, please notify the Nurse. The physical can be submitted to the nurse via email or handed into the main office

DENTAL EXAMS

Students entering 7th and 11th grade will need a completed dental form (MEH-155)  for the 2026-2027 school year. If your child is due for a checkup after September, you can notify me via email at Dnoorlanderquain@philasd.org

EARLY DISMISSALS DUE TO ILLNESS

If your child becomes ill during school hours, the nurse might determine that the child should be picked up from school.  Only listed emergency contacts or parents/guardians can pick up a child for early dismissals due to medical reasons. Ensure that your emergency contact form is updated with the main office any time your telephone number or address changes. Contact the Main Office for emergency contact forms. Per School District policy, all early dismissals must be done in the Main Office with proper identification.

GUIDELINES FOR KEEPING STUDENTS HOME FROM SCHOOL

  • Fever: 100 degrees Fahrenheit or higher, the student should remain home until free of fever for 24 hours without using fever-reducing medications.
  • Diarrhea, Vomiting: The student may return to school 24 hours after the student is free of symptoms.
  • Vomiting: Occurrence of more than one episode of vomiting and associated with other symptoms. May return to school 24 hours after the student is free of symptoms.
  • The child was in the hospital.  The child must have a note to return to school.
  • Eye infection: The child must be evaluated by the doctor and have a note to return to school.
  • The child has a rash with itching or fever. Free from rash, itching, or fever and submit a note to return to school.
  • Earache, ear drainage: The child must be evaluated by the doctor and have a note to return to school.
