When students return for hybrid learning
If your student is returning to hybrid learning, you will have to complete the COVID Testing Consent. Information is available at https://www.philasd.org/studenthealth/covidtesting/

Please remember the Pre-Covid-19 checks before sending the student to school, and if your child is experiencing any of these symptoms, please keep them home and have them evaluated. Symptoms include:
● fever of 100.4℉ or greater
● cough (not related to pre-existing condition)
● shortness of breath
● sore throat
● headache
● chills
● new onset of loss of taste or smell
If your student’s regular vaccinations (measles, mumps, rubella, etc.) are not up to date, they will not be able to return to building – no exceptions!
Please also make sure their emergency contact form is up to date.
