New Student Enrollment
GAMP welcomes all new students! We look forward to supporting and challenging you as you grow!
To complete new student enrollment, all caretakers of new students must submit the required documents by clicking this link.
- Proof of child’s age: At least one of the following: original birth certificate, valid passport, original baptismal certificate indicating the date of birth
- Proof of Residency: A deed, mortgage statement, or a complete, fully executed current lease AND one
other form of approved residency documentation, listed in the link above. If a Deed, Mortgage statement, or lease is not provided, caretakers must provide three (3) forms of approved residency documentation, listed in the link above. - EH-40 Form: Application for school admission.
- Immunization Records
- A current physical exam from the child’s doctor
- S-865 Form: Student Emergency /Medical Information
Media Release Form: GAMP concerts are recorded, and the media release form is required for students to participate in GAMP music concerts.
The main office will be open starting on Monday, August 11, from 8:00 to 3:00. Our staff will be on site to answer questions, provide support with documentation, and assist with uploading forms.
